Privacy Notice

Providing personal data: what information do we collect and why?
*Full name, status, date of birth, employment status and occupation, relationships - basic information to help me get to know you and your life.
*Address, email address, home and/or mobile number - is used to be able to contact and communicate with you regarding your sessions.
*GP details & Medical health data(provided by you) - to risk assess and determine whether therapy is a suitable and the best course of action for you. Your GP will only be contacted if I thought you were at risk of harm or needed clarification on any mental health issues.
*Life history and reasons for seeking therapy. - this is to help understand your experiences, to enable us to help you identify your difficulties and patterns of negative behaviours  and your goals from therapy. From this we plan a way forward in therapy.

Sharing of personal data:
Your data will not be sold or used for unethical reasons.  
Subject to applicable data protection law, we may required to share your personal data with:
* Sub contractors and other persons who help to provide our products and services.
*Government bodies and agencies in the UK (e.g HMRC, who may in turn share it with relevant tax authorities and with regulators)
*A legal obligation by law to do so ( i.e If you or anyone is in danger or at risk of harm, involved in acts of terrorism, major criminal activity or requested to do so by Her Majesty court Service) 

Retention of personal date: How will I store your data..
Minds at Peace Therapy will securely retain the data that you have provided to us and the records that we have used for the period of time that  we have a proven legitimate reason to do so. This will be kept for a maximum of six years from point of contact or consultation.  
In complying with GDPR Minds at Peace Therapy  will ensure that Personal Data shall be:
*Processed lawfully, fairly and in a transparent manner.
*Collected for specified, explicit and legitimate purposes.
*Adequate, relevant and limited to what is necessary.
*Accurate and, where necessary, kept up to date.
*Retained only for as long as necessary
*Processed in an appropriate manner to maintain security.

I will store all paperwork and session notes for a period of 6  years which is the requirements of my insurance company and in a locked filing cabinet.  Our mobile and laptap access are password protected.  We delete any texts or emails from you as soon as they are no longer needed or at least within one month of our work finishing. I delete automatically saved clients numbers out of my phone within 1 month of us finishing work. 

Data Access Request and Opting out:

If you believe that any information we are holding on you is incorrect or incomplete you have a right to ask for it to be deleted or rectified.  However Please email the DPO on - info@mindsatpeace.co.uk with your request.
